Tirzepatide is a 39-amino acid synthetic peptide with agonist activity at both the glucose-dependent insulinotropic polypeptide (GIP) receptor and the glucagon-like peptide-1 (GLP-1) receptor. Its structure is based on the GIP sequence and includes a C20 fatty diacid moiety that prolongs duration of action. Of the 39 amino acids, 37 are naturally occurring, while two are non-coded aminoisobutyric acid residues at positions 2 and 13. Tirzepatide has a greater affinity to GIP receptors than to GLP-1 receptors. This dual agonist behaviour has been shown to produce greater reductions of hyperglycaemia compared to selective GLP-1 receptor agonists. In May 2022 it was approved by the US FDA under the brand name Mounjaro for type 2 diabetes, and subsequently approved as Zepbound for weight management. For research use only.
